Assessment Phase

Intake


Receive self-referral, or referrals from concerned parties like parents, school, court, or other professionals

Psycho-Social Assessment


Do a psycho-social assessment (evaluation) of:

  • the client’s psychological functioning
  • in the context of their specific social environment

Individual Treatment Plan (ITP)


Develop a treatment plan based on

  • the results of the assessment
  • the Risk-Need-Responsivity-Framework
  • the input & situation of the client
  • the requirements of the referral source
Intervention Phase

Contracting


Contract with the service user based on the Individual treatment plan.

Intervention


Implement intervention/s as planned in the ITP

Most of NICRO’s behaviour modification programmes are group-based (increased learning, supportive environment, accountability, creative solutions)

Monitor and Evaluate


Monitor by the ongoing collection of data to ensure that there is progress towards the goals of the ITP.

Evaluate at set times to determine the impact, appropriateness, effectiveness, & efficiency of the interventions.

Feedback


Review against ITP with client

Where relevant - Feedback reports to referral sources (courts, schools, etc)

Reporting on compliance (courts, schools)

Aftercare Phase

Aftercare Plan


  • Plan how to sustain changes that resulted from the interventions.
  • Plan how to deal with challenges.
  • If needed, refer for further services

Aftercare Tracking


Follow up on progress at 3, 6 & 12 months.

Termination


File will be closed 12 Months after completion of services – given there is no further support or supervision needed or required by the court.
